Transcript Request Procedure

 Current CHS Students

Please follow the procedure below to request a transcript.  Please allow 10 school days for transcript to be processed.

·       Fill out transcript request form COMPLETELY (available in the Guidance Office)

You must complete a transcript request form for each college or scholarship.

·        Provide addressed envelope with postage

         Be sure you have a large enough envelope and  enough postage for all documents.

Be sure to include (if required):

·       Any additional forms

·       Letters of  Recommendation

·       Essay

 CHS Alumni

Coventry High Alumni and former students can request a formal transcript by mailing the request to Mrs. Kathy Pieczarka at the high school, or by e-mail at pieczarkakathleen@coventryschools.net.  All requests must include full name, your name when you were a student, date of birth, year(s) you attended Coventry High School, date of graduation, and the name and address of the school, business or organization to send the transcript.
